Appointment Information

Please make an appointment to see Dr Maritz at his rooms at Montana Hospital.

We do not require a referral from another doctor. Your medical aid may require a referral for reimbursement purposes.

Our receptionist may ask you to come in early to perform X-rays before the consultation. You will need to obtain a letter from us before going to the radiology department. Montana hospital has a radiology department.

All consultation fee’s need to be paid in the consultation rooms via credit card. The account statements will be mailed and patients need to liaise with their medical aid for reimbursement.

Please bring proof of identification and your medical aid card.

In the rooms you will need to fill in a form with the main member’s and the patient’s details.

During your consultation Dr Maritz will discuss your treatment options, including whether conservative treatment is necessary or an operation is required.  He may ask you to obtain further X-rays, sonar or MRI before making a decision.

Before surgery

Before surgery, there are a few things that need to be completed. These include:

  • An informed consent form needs to be signed.

  • A Form 35 needs to be signed for children under 18.

  • Hospital Authorisation needs to be obtained. We will provide you with the details to obtain authorisation. Please check that all the details given to you by the medical aid match our details that we supplied to you.

  • A Hospital pre-admissions needs to be completed.

  • Certain medical aids or medical agreements may require a visit to the anaesthetist or specialist physician for pre-operative checks. Adequate time should be allowed for this process.

  • The authorisation number must be mailed to the rooms or WhatsApp’ed to the rooms as soon as possible. At least one day before surgery.

  • All the required forms must be signed and mailed to the rooms or WhatsApp’ed to the rooms a day before surgery.

  • You will receive pre-operative documentation that will explain all you need to know.

  • An anaesthesia form needs to be completed before surgery.

In hospital

Depending on the operation you may require physiotherapy in hospital. Some operations also require orthotic services and equipment such as braces, crutches and stockings. Special cases may require specialist physician care.

After surgery

You are provided with discharge instructions. This is a very important document and should be read and adhered to.

Please note:

  • You will receive a medical certificate that can be used for sick leave purposes.
  • Please check that you receive all your pain medication and other pills as required upon discharge.
  • You may be required to have physiotherapy out of hospital.
  • Our practice has follow up consultations 1-3 weeks after surgery. ( Check your discharge instructions.)
  • Replacement surgery also has a follow up consultation after 3 months.
  • Contact the rooms if you have any questions after surgery.
  • In certain circumstances you will be transferred to another facility such as a step down or frail care.  This can only be arranged once admitted to hospital. Medical authorisation is required for this and this may delay your discharge and prolong your stay in hospital.
